For the PDF forms, you may mail your registration and payment or fax your registration and pay by telephone with your credit card.  Call 843-918-1162 to provide your card number, then fax your completed registration form to 843-918-1137.  Please include your telephone number on the registration. 

The City of Myrtle Beach, Lack’s Beach Service, the Myrtle Beach Area Chamber of Commerce and Horry County welcome the competition, which is expected to bring 1,500-2,000 adult competitors and 300-500 junior competitors and their families.  The Crown Reef Resort is the host hotel, and the competition will be held on the beach between Damon’s Restaurant and Springmaid Pier.  Visit www.usla.org or www.lacksbeachlifeguards.com for more information.
